If you batch restore, I personally just choose App Only. It's a pain to re-sign in to various apps, but coming from the DX, restoring Apps+Data caused problems, including a never ending sync icon in the notification bar that gulped down battery juice. I don't know if this happens on the Bolt, but I've played it safe in this respect.
Question: "Do I have to wipe data before I flash this?"
Answer: "Only if you want zero problems."
Btango, I love. You're guide is so awesome for a noob like me. My only fear now is flashing a radio. Although, I haven't watched the video yet. . .do,you flash radios before roms or roms before radios or does it not matter?

I flash the ROM in Clockwork Recovery, then instead of letting it reboot, I pull the battery and boot into hboot (bootloader) and flash the radio.
Pull battery, reinsert, hold volume down while pressing power until hboot comes up.

Just make sure that, before you go to reboot into recovery to flash the ROM, that you have the correct radio file on the root of your sdcard (not in a folder), and that it's named correctly.

How necessary is it to do the format steps on the SD card? Specifically the /data and /system folders. I have titanium backup and lost a lot of app data I thought was backed up when I formatted these folders on the SD card.

I don't know. I never use Titanium to restore app data. In the past, I was burned doing that on my DX. Caused a never ending sync loop that tore through battery juice fast. So now I just use Titanium to restore the app only.

It's always the same name. PG05IMG.zip. The thing that gets some people is that some versions of Windows hide known extensions, such as .zip Many were naming the file manually to PG05IMG.zip, not knowing that since it was already a zip file, it meant that the file's new name became PG05IMG.zip.zip
Most ROM developers will post a link to the recommended radio file in the thread where they have the link for downloading the ROM.
