Phone Won't write or keep changes

So my friend's thunderbolt is having odd issues.

His phone is rooted and does not have a custom ROM installed.

    The Issue:
A little over a month ago he began having issues with the phone to the point where it is unusable. Any changes that are made to the phone, any apps downloaded/installed, etc.. upon a reboot are gone. Almost as if a snapshot has been created and is restored after rebooting. Also, when he attempts to reboots the device from within the phone... the device hangs at the HTC boot logo. The only way to get past it is to pull the battery, after doing so the phone boots right up into the OS (without keeping any changes made prior to rebooting).

    Troubleshooting steps taken:
His phone has clockwork recovery installed. We've tried installing custom ROM's (cyanogen, BAMF), wiping data/cache/dalvik etc., restoring the device back to factory settings... And every time when the boots up for the first time (after installing custom ROM's and performing data wipes, etc), the phone hangs at the white HTC boot logo screen. We have let the phone sit for over 20-30 minutes at this screen and nothing happens. When we pull the battery the phone boots right up; however it is as if we have not tried anything (like that snapshot has been restored).

Has anyone seen or heard of something like this, or have any ideas on how to possibly fix it? If you need clarification or more information, please let me know. I currently do not have the phone in front of me, but I will this weekend.
