I'm on step 4 now. The phone is powered off and charging. I'll take a screen shot of the Battery Calibration App voltage once I power into recovery, wipe battery stats, and power on.
Phone is on and calibrated. Here is two screenshots...first one is after step one, second is after step 4 and wiping battery stats-
I'll report back periodically with screenshots during normal useage discharge.
What battery are you using? Stock, Rezound, etc.?
I'm on a Seidio 1750. My phone is charging via usb and at 59% I'm at 3925mV.
It is very possible that there is a hardware issue with your phone. I've read, either on this forum or TeamBAMF, that one or 2 people had charging issues.

I have the Rezound in currently.
Looks like you are on 4Ever Cubed. With the Rezound battery and moderate usage, you should get at least a day and a 1/2 before needing to recharge.
With my Seidio, on SkyRaider 1.3, I can go over 2 days.

Yep, I'm on BAMF forever cubed 1.0.8
It's a great ROM, just wish my battery voltage reading would cooperate.
Last screen shot doesn't look like you started out at 100%.
Try bump charging. Plug in, wait until green, unplug and wait a few seconds, plug back in, etc. Do this until you get a green light within seconds after plugging in. Once done phone should be at 100%. Leave it plugged in. Immediately use the Battery Calibration app and wipe stats again.
After you do that, let the phone rundown on it's own and recharge. Do this at least twice. Turn off any power saving options and don't force the phone to drain battery. Use it normally. And do this with the one battery you have in there. If you keep swapping batteries, stats will be off.
Other than this, I can't think of anything else that might help. Since you are using a BAMF ROM you can try checking here.

That's precisely my problem...I can't.
The issue is, the phone will not EVER get to 100%. The LED never turns green because the phone doesn't think its charged, I guess. I can bump charge all day long, it always stays amber. I have posts over on TeamBAMF, and no one has a solution.
I don't think its a hardware issue, as it started around the same time as the OTA update came.
